Uygulama ile y\u00fcksek hassasl\u0131kta analizler yap\u0131labilecek ve kanserin ki\u015fiye \u00f6zg\u00fc DNA\u2019s\u0131 \u00e7\u0131kar\u0131lacak.<\/p>\n<p>BU\u00dc T\u0131p Fak\u00fcltesi Genetik Hastal\u0131klar Tan\u0131 Merkezi\u2019nde t\u00fcm\u00f6r dokudan DNA izolasyonu ile uygulanan yeni nesil dizileme tekni\u011fine ek olarak likit biyopsi teknolojisi ile kandan t\u00fcm\u00f6r kaynakl\u0131 DNA\u2019lar (cell-free DNA) elde ederek yeni nesil dizileme tekniklerinin uygulanmas\u0131 art\u0131k m\u00fcmk\u00fcn. Yap\u0131lacak i\u015flem sayesinde t\u00fcm\u00f6rl\u00fc dokudan kan dola\u015f\u0131m\u0131na kar\u0131\u015fan DNA par\u00e7ac\u0131klar\u0131 \u201cGenereader\u201d sistemi ile y\u00fcksek hassasl\u0131kta analiz edilerek, kanserin ki\u015fiye \u00f6zg\u00fc DNA haritas\u0131 \u00e7\u0131kar\u0131labiliyor. G\u00fcncel bir uygulama metodu olan ki\u015fiye \u00f6zg\u00fc tedavi \u00e7er\u00e7evesinde bu iki yeni teknoloji, T\u0131p Fak\u00fcltesinde g\u00f6rev yapan uzman doktorlar\u0131n birikimiyle kanser hastalar\u0131na umut vaat ediyor.<\/p>\n<p>Yap\u0131lan uygulamalar hakk\u0131nda bilgi veren Genetik Hastal\u0131klar Tan\u0131 Merkezi Mesul M\u00fcd\u00fcr\u00fc Do\u00e7. Dr. \u015eehime G\u00fcls\u00fcn Temel, hem hedefe y\u00f6nelik hot-spot panelleri hem de t\u00fcm gen dizi panellerini kapsayan bu iki i\u015flemin birlikte kullan\u0131lmas\u0131yla  hastalar\u0131n tedavisine daha b\u00fct\u00fcnc\u00fcl bir katk\u0131 sa\u011fland\u0131\u011f\u0131n\u0131 aktard\u0131. Temel \u015f\u00f6yle konu\u015ftu:<\/p>\n<p>\u201cKanser tan\u0131s\u0131 alm\u0131\u015f ki\u015filerin dokular\u0131nda ve likid \u00f6rneklerinde (periferik kan, t\u00fckr\u00fck gibi) hedefe y\u00f6nelik yeni nesil dizileme tekni\u011fi ile hot-spot klinik anlam\u0131 olan mutasyonlar\u0131 i\u00e7eren b\u00fcy\u00fck bir panel kullanarak hastan\u0131n tedavi s\u00fcrecinin planlanmas\u0131na katk\u0131 sa\u011flamaktay\u0131z. Ayr\u0131ca e\u011fer tedaviye y\u00f6nelik bir varyasyon tespit edilemezse, bu kez likit biyopsi y\u00f6ntemi sayesinde 56 geni i\u00e7eren geni\u015f bir panel ile yeni nesil dizileme tekni\u011fi ile t\u00fcm genlerin dizi analizi m\u00fcmk\u00fcn olmaktad\u0131r. Likit biyopsi, kanser tan\u0131s\u0131 alm\u0131\u015f tedavi s\u00fcrecindeki hastadan al\u0131nan 30 ml kan kullan\u0131larak tedavinin hastaya \u00f6zg\u00fc planlanmas\u0131, izlenmesi, do\u011fru ila\u00e7 se\u00e7imi yap\u0131labilmesi amac\u0131yla kullan\u0131lmaktad\u0131r. Di\u011fer taraftan likit biyopsi giri\u015fimsel olmayan bir y\u00f6ntem olmas\u0131 nedeniyle hasta refah\u0131 a\u00e7\u0131s\u0131ndan oldu\u011fu gibi doku biyopsilerinin yetersiz oldu\u011fu durumlarda da hastaya tedavi planlanmas\u0131 ve takibi a\u00e7\u0131s\u0131ndan \u00e7ok b\u00fcy\u00fck bir fayda sa\u011flamaktad\u0131r.\u201d<\/p>\n<p>&#8220;Y\u00fcksek hassasiyet i\u00e7eren bir teknoloji&#8221;<\/p>\n<p>Tan\u0131 Merkezi Sorumlu Hekimi Do\u00e7. Dr. \u015eebnem \u00d6zemri Sa\u011f ise merkezde Damlac\u0131kl\u0131 Dijital PCR (ddPCR), Minimal ResidualDisease (MRD) tespiti hassasiyetinde p210 majortranslokasyonunun da analiz edilmeye ba\u015fland\u0131\u011f\u0131n\u0131 s\u00f6yledi. Damlac\u0131kl\u0131 Dijital PCR ile analiz edilecek \u00f6rne\u011fi 20 bin damlac\u0131\u011fa b\u00f6l\u00fc\u015ft\u00fcrd\u00fcklerini vurgulayan Sa\u011f, \u201cPCR b\u00f6l\u00fc\u015ft\u00fcr\u00fclen her damlac\u0131k i\u00e7inde olu\u015fan reaksiyonu mutlak kantitatif olarak \u00f6l\u00e7me prensibi ile \u00e7al\u0131\u015f\u0131r. Damlac\u0131klara b\u00f6lme prensibi sayesinde hassasiyet y\u00fckselir ve b\u00f6ylece nadir mutasyonlar\u0131 tespit etme imk\u00e2n\u0131 sa\u011flar. MRD tespitinde en k\u00fc\u00e7\u00fck kal\u0131nt\u0131lar\u0131 tespit etmek esast\u0131r. Bunun i\u00e7in en y\u00fcksek hassasiyete ihtiya\u00e7 vard\u0131r. ddPCR ile MRD tespiti yapmak, ba\u015fka tekniklerle tespit edilemeyecek kadar az olan kal\u0131nt\u0131lar\u0131n fark edilmesine olanak sa\u011flar.
